This form is for Special Events (i.e. funerals, weddings, concerts, etc.) taking place in the Sanctuary or Fellowship Hall.
This form must be fully completed and submitted at least 4 days prior to most events. When possible, for weddings and concerts it must be completed and submitted at least 2 weeks prior.

All music and/or visual media must be submitted at least 3 days prior to the event. All files should be submitted via flash drive: audio files in MP3 format; video files in MP4 format.
